Liability waiver & release

Assumption of risk and release for anyone who rides, handles, or is around the horses — visitors included. Required before you get near a horse. Minors are covered by a parent or guardian signature.

02Boarders

Boarding agreement

The board contract — horse details, your info, what's included, payment terms, and the care/custody terms. Signed once when your horse moves in.

03Riders

Lesson registration & waiver

Sign up a rider — kid or adult. Experience, helmet acknowledgment, and the liability release all in one. One per rider, then you're set for the season.

04Minors

Youth & camp consent

For Summer Camp Days — parental consent, medical info, emergency contacts, and authorization to treat. One per child, per season.

05Training

Training intake agreement

Drop a horse for 30/60/90-day training or colt starting. Goals, history, vices, vet/farrier, and the training terms — including the daily-care and release language.

06Owners

Emergency vet authorization

Tells us what to do if your horse needs a vet and we can't reach you — who to call, your spending limit, and standing instructions. Keep it on file for every horse.

Horse health & Coggins record

Vaccinations, negative Coggins, deworming, shoeing schedule, feed, and any conditions or medications. So we care for your horse right from day one.

You can save your own copy at the end of each form (your phone or browser will save it as a PDF).

These forms exist because of our insurance and Tennessee's equine-activity law — they protect you, your horse, and us. The warning notice required by TCA Title 44, Chapter 20 appears on every one.
