I authorize Gracewood Acres, when my horse needs urgent veterinary attention and I cannot be reached after reasonable effort, to contact the veterinarian above (or the nearest available licensed veterinarian) and authorize examination and treatment on my behalf, following the instructions I've given on this form.

I understand Gracewood Acres will always try to reach me first and will only act on these standing instructions when time or my unavailability requires it. I accept financial responsibility for all veterinary costs and agree to reimburse Gracewood Acres for any vet, medication, or hauling expenses they advance on my behalf.

Acting in good faith on this authorization, Gracewood Acres is not liable for the outcome of care obtained or decisions made under it, except in the case of gross negligence. This authorization stays in effect while my horse is at Gracewood Acres, unless I revoke it in writing.

I authorize emergency veterinary care per my instructions above when I can't be reached. I accept financial responsibility and will reimburse costs advanced on my behalf.
